  • The test of “Sikh History & Religion” will be only qualifying in nature and marks secured therein will not be counted for the purpose of gradation in merit list.
  • Candidates are advised to complete the formalities well in time to avoid last date rush on web server. All Steps of Application Form / Registration Form Process should be completed before closing date as per the Advertisement. Incomplete application/registration will be rejected automatically without any intimation.
  • Read carefully Advertisement, Information Brochure, Important Information, How to apply and key dates before filling up the Online Application Form / Registration Form.
  • Before you proceed to register yourself for entrance examination you must ensure that you have read and understood the eligibility criteria for each course for which you intend to take the admission. Kindly fill this online form only if you meet the eligibility as your form can be rejected at any stage due to ineligibility.
  • You should have a valid personal email ID. In case, a candidate does not have a valid personal email ID, he/she should create his/her new email ID before applying Online. You should keep checking the inbox or even the spam box of this email id regularly during the admission process as the institute may send intimations for written test etc. through this.
  • Create your login account by filling your basic details on Online Admission Portal Then you will get SMS as well as email intimating about your login details.
  • Login into your account and then fill the online application form carefully in Step 1. Amendment / rectification / change / editing are NOT allowed after submitting the Application form and payment. BE CAREFUL while filling your details in Online Application Form.
  • For your confirmation, you can save and preview the form before final submission of the form. Before final submission of online Application form, read the declaration carefully and give your consent on it. So, you must check your information details carefully before final submission of your application form. Information once submitted will be treated as final and no request related to correction/changes will be entertained.
  • In step 2, you are required to upload your scanned image of recently taken photograph and signature in JPG format. The digital size of the photo file and the digital size of the signature should be between 8 KB and 50 KB as per specification mentioned in the prospectus, failing which you will not be able to complete your application. Candidate is required to upload his/her signature and latest passport size photograph with following specifications on the Application Form & Admit Card (for Sikh Minority Quota candidates only).
    • A photograph of minimum 35X45 mm with at least 75% coverage of face & head of the candidate.
    • A caption indicating name of the candidate and date of taking photograph should be there at the bottom of the photo.
    • The Photograph needs to display full front view of the face. Please look directly into the camera with a neutral expression.
    • Please avoid photograph with reflection or shadow on the face with red eyes.
  • In step 3, Applicant can click on “To Pay through Net Banking/Credit Card/Debit Card” for online payment through Net-Banking / Credit Card / Debit Card. Kindly note that the Bank charges will be extra as per Bank rules. On successful payment, the candidate will receive SMS about the confirmation of the fee receipt on the registered mobile number. No written acknowledgement will be issued for the receipt of the application form. Fee paid in any other bank or in any other format (Cheque, Postal Order, Demand Draft etc.) will not be accepted.
  • In step 4, Applicant can take print of Application Form and Admit Card (for Sikh Minority Quota candidates only) once his/her payment is approved by Bank. Click on Print Application Form and Admit Card link to take print out. The Applicant is required to print at least 2 copies of the form and get them signed. Candidate has to send one of the application form's hard copy along with the supporting document to the institute either by hand or through Registered Post on or before 10/02/2017 by 03:00 p.m.
  • Kindly note that the candidate is also required to download his/her Admit Card (for Sikh Minority Quota candidates only) and get it printed (minimum 2 copies to be safer) and keep that for the entry in Examination Centre (Sikh History & Religion Exam) and in the Counseling Hall. Entry without Admit card is not allowed in any case.
  • The print copy of application form must be accompanied by the following:
    • Affidavit in the form at Appendix B-1/B-2 that he/she belongs to Sikh community (for Sikh Minority Quota candidates only).
    • For those who do not presently bear the word “Singh” or “Kaur” with their name, an affidavit that he/she has changed his/her name (in Appendix-C).
    • Two self-addressed envelops.
    • Copy of AIPGDEE-2017 Result Card.
    • Institute’s copy of Bank Challan, in case the payment is made through bank challan. The application fee once paid shall not be refunded.
    • Two passport size recent colour photographs (from the same negative), kept inside an envelop.
  • Once the application form along with its enclosures has been submitted, the applicant will not be permitted to make any alteration or preference of course, etc, as originally mentioned in the application form.
  • Applications of candidates producing false or fabricated information will not be considered and candidates may be further debarred from appearing in any future examinations. 
  • Candidate shall fill online ADMISSION APPLICATION FORM only once. As multiple Id’s of same applicant create technical problems.
  • Incomplete admission application form or without requisite fee will not be entertained and will be rejected without any intimation to the candidate(s).
  • Note: Sending admission form through a courier service is absolutely not advised because no courier service delivers at the institute office. Sending the admission form through speed post should also be avoided as some times serious delay is likely to occur, for which the candidate himself /herself shall be responsible.
  • An application form which does not reach the office of the Registrar, Sri Guru Ram Das University of Health Sciences, Sri Amritsar within the prescribed date and time shall be summarily rejected. In such a situation, the examination fee shall not be refunded.
